Abstract
Background:Psoriasis is a skin condition distinguished by inflammation, hyperproliferation of epidermal cells, & a noticeable influx of macrophages, neutrophils, & T lymphocytes. T-helper 1 (Th1) cells were once thought to be the mediating factor in psoriatic inflammation; however, more & more recent research suggests that immunological responses by recently identified Th17 cells have a role in the pathophysiology of psoriasis. Objective: To study the influence of UVB on the serum level of P19 subunit of IL-23 in patients with psoriasis. Patients and Methods: Every patient was chosen at random from the Suez Canal University Hospital's dermatology outpatient clinics. Before their involvement in this study, all patients provided their informed permission. The Suez Canal University Hospital's College of Medicine's Ethical Committee gave the study its approval. A thorough history taking & a general & dermatological clinical examination were performed on each patient. Blood samples were obtained at baseline & one-week post-treatment (week 12) to test P19 subunit of IL-23 level & perform a blind clinical assessment using PASI score calculation. Three factors are used to assess the severity of psoriasis: E stands for erythema; I for infiltration; & D for desquamation.Results: In our study, 24(80%) of patients were male & 6(20%) patients were female. The mean ± SD of the age of patients is 33.27 ± 10.27 years (range 19-45). Most of the patients 21(70%) were smokers with BMI of (24.18 ± 3.08) ranging from 21 to 33 kg/m2. Regarding psoriasis area & severity, the PASI score was 14.33±4.24 ranging from 10 to 16. Also, 8(26.7%) of cases had positive family history & 15(50%) of patients had stationary courses, 3 (10%) had regressive courses & 12(40%) had progressive course. Half of patients 15(50%) had psoriasis at the head & upper limb, while 18(60%) had disease at trunk & 21(70%) had the disease at the lower limb. Patients had a mean duration of disease of 7.16 ± 2.34 years ranging from 2 to 20 years & mean age of disease onset of 34.74 ± 11.07 ranged from 19 to 45 years old. Conclusion: we found that, UVB treatment resulted in a reduction in P19 subunit of IL23 & also PASI score. Also, there were significant correlations between P19 subunit of IL23, the duration of the disease & PASI score.
